PRE-EMPLOYMENT REQUIREMENTS 

FOR

EMPLOYMENSUBSTITUTING | VOLUNTEERING | MENTORING | COACHING

WITHIN THE NORWALK CITY SCHOOL DISTRICT

Check once completed:

o  Norwalk City School District Application.  All positions require an application.

You may attach/upload the following documents to the application: as pdfs (listed below).

 

o  Background Checks.  You will need to obtain both background checks (BCI and FBI) for your initial employment, and if necessary, to obtain your credential through the Ohio Dept. of Education. If you have a background check less than one year old, it MAY be acceptable, the Superintendent will make that determination.

 

Background checks may be obtained through our district’s Central Office, 134 Benedict Ave. Background checks are processed by appointment only on Mondays and Wednesdays between the hours of 2:00-3:45 p.m. To schedule an appointment, please call  Kim McConegly at 419-660-1805, or emailing her amcconeglyk@norwalktruckers.net. You will be asked to acknowledge that you understand that any criminal history will be released to the Norwalk City School District and/or the Ohio Dept. of Education.  The cost for both background checks is $56 if working for Norwalk City School District; CASHMONEY ORDER, or CREDIT CARD and you must have a valid drivers license or state I.D.

 

o Highest Degree: Diploma/GED Certificate/Official College Transcripts – upload with application

 

o  Credential. You will need to obtain a credential through the Ohio Dept. of Education for the following positions: teacher, substitute teacher, educational aide, substitute educational aide, paid or volunteer coach.

                      

To apply for a credential go to the Ohio Dept. of Education’s website (https://education.ohio.gov) type in the appropriate credential in the search box (i.e., substitute teacher, educational aide permit, coaches: pupil activity permit).  There are instructions on their website to help you obtain the proper credential.  Use 044560 to search for the Supt. Signature – Norwalk City School District.

REQUIREMENTS FOR ALL COACHES (PAID OR VOLUNTEER)


1.

BCI Background Check (1) - Date of Record Check:                                          

(Background checks are processed by appointment only on M & W 2-3:45. To schedule an appointment, please call 419-660-1803. Cost: $27 (exact change is appreciated) CASH/Money Order/credit card & picture ID

FBI Background Check (2) - Date of Record Check:                                            

Background checks are processed by appointment only on M & W 2-3:45. To schedule an appointment, please call 419-660-1803. Cost: $29 (exact change is appreciated) CASH/Money Order/credit card & picture ID

 

3.

First Aid for Coaches – Approved course through Fisher-Titus Medical Center, or other approved provider.

4.

CPR Certification - Date of Completion:                                      

Proof of completion should be given to the Athletic Director at NHS.

 

5.

Sudden Cardiac Arrest Course – Date of Completion:

Fundamentals of Coaching - Date of Completion:                      

Go to www.nfhslearn.com to take the course.  When finished print certificate and deliver to Athletic Director at NHS.

 

7.

Concussion Education Class - Date of Completion:____________

Go to www.nfhslearn.com to take the course or through the CDC. When finished print certificate and deliver to Athletic Director at NHS.

 

8.

Pupil Activity Permit issued by  – Issue Date:                    

After completion of 1-7 above, apply through or create a SAFE account for your Pupil Activity Permit:  https://safe.ode.state.oh.us/portal. Use 044560 for Supt. Signature for Norwalk City School District. 

 

9.

Paid Coaches – complete payroll forms – see Karen Wiedemann, Treasurer’s Office, 134 Benedict Ave. (Must sign and return contract once approved by the Board of Education in order to be paid.)

 

(1)    BCI & FBI necessary prior to initial employment

(2)    Then, FBI only, every 5 years thereafter

Coaches are responsible for all costs incurred while completing coaching requirements. The Norwalk City School District’s Athletic Departments do not reimburse coaches for incurred costs. However, in some cases individual booster groups may reimburse coaches within their program for these costs. This arrangement is between each individual coach and that particular booster organization.
