Guidelines for Medication Administration 2023-2024
New Medication Administration Guidelines for the 2023 -2024 school year are outlined below. The Medication Administration Record (MAR) form is located as a link just below this information, the NCSD App, or it can be picked up at any NCSD building. While these guidelines may be inconvenient for some, the safety of your child and all of our students is of utmost importance!
High School and Middle School Students (Grades 7-12):
Prescription medications (any medication ordered by a physician)
The completed Medication Administration Form and prescription medication must be brought to the office by a parent or legal guardian. The medication must be in its original container with the prescribing information on it. The school nurse, or authorized staff member, will inventory the medication with the parent present. This includes verifying that the Medication Administration Form is complete and that the information on the form matches the information on the medication label. Medication will be counted and recorded, then signed by the school nurse, or authorized staff member, and the parent. The Form must be signed by BOTH the physician and the parent. This procedure will be done with each refill as well.
If your child is diabetic, we need to have the Medication Administration Form along with a copy of your child’s care plan (sliding scale for insulin coverage and carb ratio), even if your child is self-sufficient with their testing and insulin administration. In the event your child is unable to treat themselves, we must have all orders should we need to test them and administer their insulin for them. You may send this in with your child either before school starts or on the first day of school. Please send a glucose meter, extra test strips, batteries, sensors, and extra insulin, to keep in the office.
If your child needs to carry an EpiPen or inhaler, we need to have the Medication Administration Form along with a copy of your child’s care plan (usually provided by their physician), in order for your child to carry their medication with them during the day. We still need to inventory the medication and keep records of all physician’s orders, in the event we need to provide care for your child. We also recommend an extra EpiPen or inhaler be kept in the office.
Any prescription medication (with the exception of EpiPens and inhalers) that is left over will need to be picked up by a parent. Any medication that has not been picked up by the last day of school will be disposed of appropriately.
Non-prescription/over-the-counter medications (any medication not ordered by a physician)
Please fill out the Medication Administration Form. All requested information about the medication must be provided on the form. Since this is a non-prescription medication, the physician’s signature is not required, however, this form must be signed by a parent or legal guardian. The parent can bring the form and medication to the office, or you may send the form and medication with your student. As with prescription medications, we will inventory the medication, and the student will sign with the school nurse or authorized staff member. A medication count, however, will not be necessary for non-prescription medications. The non-prescription medications must arrive in its original container. The box must be new, unopened, with the original seal, or it will not be accepted.
Any non-prescription medication that is left over at the end of the school year will be sent home with your student. If you would prefer to pick it up in the office or have us dispose of it appropriately, please notify the office.
Maplehurst, Pleasant, League, and Main Street Students (Grades Pre-k thru 6) :
Prescription medications (any medication ordered by a physician)

If your child requires an EpiPen or inhaler, please bring in the completed Medication Administration Form, along with your child’s care plan (usually provided by their physician), and medication, to the office. We will inventory the medication and sign with the parent. The information on the Medication Administration Form must match the information on the medication label. If your child needs to carry this medication with them at school, then your child’s physician must indicate that in the space provided on the Medication Administration Form. Please do not send this medication to school with your child. Please bring the form and medication to the office first, to be inventoried, before allowing your child to carry it. For the safety of your child and all other students, it is important that we have this information in the office.

Please note that we will not administer any prescription or non-prescription medication that is expired!
