The Norwalk City School District, like many districts across Ohio, is in the process of developing an educational plan for the 2021-2022 school year.  As a part of that plan, the registration for your kindergarten student may be completed with some positive changes prior to school beginning this fall.  Registration will now be completed using an on-line registration system, FinalForms. Norwalk City Schools has collaborated with FinalForms, an online forms and data management service, to simplify the back-to-school process for our families while reducing our paper usage. FinalForms allows you to complete and sign enrollment for your child or children with ease.  

The FinalForms process is currently under development and should be operational soon.  However, you may select dates from the list below to sign up for screening. To schedule your child’s appointment for kindergarten screening, click here: Schedule Screening Date .  You then will be directed to select a screening time and date below for your appointment.

  • April 1, 2020 at the Norwalk High School Fisher Titus Center LGI room from 8:00am-3:00pm
  • April 12, 2020 at the Norwalk High School Fisher Titus Center LGI room from 8:00am-3:00pm
  • April 20, 2020 at 195 St. Mary’s Street Maplehurst Elementary from 4:00-7:30pm

Once a date has been selected, please bring the following documents with you to your appointment and we can make copies for you:
•     Birth Certificate
•     Immunization Records
•Custody Paperwork (if applicable)
2 Forms of Proof of Residency (Water Statement/ Gas or Electric Bill)
Driver's License (ID)

It is important to know that the enrollment process will not be complete without the above documents. Welcome to Maplehurst Elementary School


Maplehurst Elementary is located at 195 St. Mary's Street near the northeastern edge of Norwalk and serves over 500 students in pre-school, Kindergarten and 1st grade throughout the Norwalk community. The school has a staff of 24 full-time classroom teachers, 8 full or part time support teachers, and 8 full- or part-time classified employees.

Maplehurst was built in 1953. The original structure housed seven classrooms, offices, and a multipurpose room. In 1956, the two story wing of the building was erected, adding an additional fifteen classrooms. The summer of 1989 saw the addition of a modular unit on the site, adding two more classrooms. Recent additions of an outdoor fitness center, playground equipment and soccer fields have provided excellent physical education and recreation facilities for school and community use.
